In determining whether a lottery winner qualifies for the prize, a court must make specific findings as to whether the lottery winner met the requirements under Subsections “b” and “c” of this section. The court order must state that the lottery winner meets the qualifications for the prize as set forth in the lottery’s rules and regulations. The court order must identify the prize payment or payments assigned to the winning player, the dates assigned to the prize payment, and the gross amount of annual payments before taxes. It also must specify the name of the prize winner as it appears on the lottery claim form.
